Breeding/Pregnancy

I have a male & female rabbit (7mo. old). They had babies on April 8. After separating them, I had the male neutered 4 days later. I obviously didn't separate them in time because they had 3 more babies on May 8. Yesterday (June 14), the female became somewhat aggressive and began building a nest. Her nipples appear to be larger and it appears she is pregnant. I'm wondering several things: 1) is it possible that the vet didn't neuter the male correctly 2) The babies would have been between 5-6 weeks old -- can they mate when they are that young? 3) What about false pregnancy?
