Dog vomiting a foamy yelow discharge

My dog is a female age 10 and she is staggering around and not able to walk far with out falling. She is vomiting a yellow foamy discharge which i believe to be bile and it has happened twice in the last 15 min. Also, she has been urinating on the floor, it puddles underneath her when she is laying down, she doesn't know she is doing it. She is just laying around next to me and she is scared what should I do?

Symptoms: Head Tilt, Lethargy, Vomiting, Collapse
